Essen Health Care has an immediate opening for a Case Coordinator, Medical Assistant to join our new Chronic Care Management team.

 What you’ll be doing

o   Outreach to patients with chronic conditions to enroll them in the Chronic Care (CCM) Program

o   Provide very basic education on chronic conditions to patients

o   Make appointments for patient for office visits, bloodwork, labs and imaging. Explain reason for referrals and appointments,

o   Assist patients with other chronic care related needs including linkages to community resources

o   Escalate clinical calls to Registered Nurses or other providers

o   Follow up with patients who are non-compliant as needed

 Working Location: 1436 Williamsbridge Road Bronx NY

Schedule: Monday – Friday 9AM-5PM


  • Excellent communication skills
  • Computer literacy
  • Strong customer service skills


  • Bilingual (English/Spanish) Speaking
  • Certified Medical Assistant
  • 1-year experience in a health care setting
