EssenMed House calls, New York’s largest house calls provider is seeking a Registered Nurse for a Case Management opportunity for our Managed Long Term care program. This program is a Managed Long-Term Care (MLTC) Plan that was created to our patients to continue to stay at home, and in your community, for as long as possible.
What you’ll be doing
As a Registered Nurse, you will be responsible for creating and coordinating plans of care for patients and will be responsible for the assessment of member needs and identification of solutions that promote high quality and cost effective health care services
Manage and provide education, chronic disease management and care to patients
Develop, monitor and evaluate care of our patients and collaborate with medical team.
Conduct assessment of patient and reassessments and will discuss findings and work closely with patient, family and physician, making referrals accordingly using community resources to ensure optimal member functioning and cost effective outcomes
Manages providers, members, team, or care manager generated requests for medical services and renders clinical determinations in accordance with EssenMed House Calls policies as well as applicable state and federal regulations.
Reviews planned, in process, or completed health care services to ensure medical necessity and effectiveness according to evidence-based criteria
Able to match patient care needs to available resources at the appropriate level of care and make adjustments if necessary
Consults with Medical Director to determine the appropriate next steps for services that require further review
Assist with following:
Collaboration and communication with patient’s primary care provider, specialist
Coordination and expedited implementation of necessary additional home care needs including skilled nursing, physical/occupational therapy, and durable medical equipment & supplies
Medication reconciliation & adherence
Patient & family education
Prescription writing, orders, refills and home-delivery
Education: Graduate of an accredited nursing program (Bachelors BSN)
Certification: Current unrestricted NYS RN license to practice professional nursing with no restrictions
Experience in long-term care, home health, hospice, public health or assisted living
Experience with arranging community resources
Working knowledge of Microsoft Excel, Power Point, and Word required
Demonstrates passion and commitment to excellence
Strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner
Possesses basic computer skills and demonstrates a willingness to learn more advanced skills
