Essen’s Health Home Care Coordination
Program
Essen’s Health Home care coordination program provides comprehensive services to our Medicaid eligible members with complex medical, behavioral and social needs. Each member is assigned to a Care Coordinator who will assist with removing barriers to improve the member’s quality care. Also, our Care Management team will help the members get the proper health and supportive services to improve their positive health outcome and independence.
We assist Medicaid eligible members with one or more chronic conditions:
- 21 years old or older
- HIV/AIDS
- Severe Mental Illness (SMI)
- Sickle Cell Anemia
- Substance Abuse
- Diabetes
- Hypertension
- Hyperlipidemia
- Asthma
Our Services include:
- Connect to primary care, subspecialty services, mental health & substance abuse outpatient program
- Access to community and supportive services, including food assistance, housing, legal, and outpatient programs.
